Fees and Financial Policies

Payment is due at time of service. The initial visit charge is $240, and follow up visits are $120.
Insurance: I do not bill insurance. If you have insurance that covers service at this office I will provide a bill to submit to your insurance company for reimbursement. If you require additional reports or documentation, a preparation fee will apply.
Worker’s Compensation: I do not handle Worker Compensation cases.
Medicare: Medicare does not cover most of the services provided in this office. I request payment at time of service. I will provide you with a diagnosis code and receipt for services rendered that you can submit to your secondary insurance if applicable.
Missed appointments: Please let me know at least 24 hours in advance if you need to reschedule your appointment. I charge $60 for missed appointments, cancellation, or rescheduling with less than 24-hour notice.
Returned check fee: There is a $25 charge for returned checks.
